Table of contents, update leaves out several headers

Operating System: Mac OS X 10.5 (Leopard)

when I generate a Table of contents from Headers there is always parts missing.
Introduction is always missing
several 3rd level headings are missing.

same document opened in word 08 on windows version will update correctly?

any suggestions?

I assume you mean you generate the Table of Contents from Heading
styles, not Headers.

Are you using the built-in Heading styles, or your own?

When you hit Option-F9 to show the field code for the TOC, what does it say?

Is the doc confidential, or can you send it to me to test?

And it's Word 2007 on Windows, just FYI.

I did mean I generated the Table of Contents from Heading

field code reads (TOC\o"1-3\h\z\u)
no the doc is not confidential but i am not sure how to post it here
when doc is sent to other word users the TOC updates properly, these users have windows systems not MACs

my disk is for Microsoft office 2008, I did not look at the word version, i assumed it was 08 also

Are any of your headings in text boxes, by chance? that could cause
it. If that's not it--

You'll have to email me the doc offline--use daiya at mvps period org
(note: daiya with an I (eye), not L (ell))--put WORD MVP in the subject
so I catch it in the spam. Give me several specific examples of which
headings are not showing up in the TOC for you, please, so we can figure
out whether the problem is something in MacWord or just on your Mac.

Your MacWord is Word 2008, yes. But Windows does not have a Word 2008,
so I was correcting your "opened in Word 08 on windows version".

Was the Windows version the very new version, with the Ribbon and
totally new interface, or an older version that still has File, Edit,
View, Insert menus?
