Task Approval Hierarchy

Hi, I am having a hierarchy as following

Group Program Manager
-Program Manager
--Project Manager
---Team Member

If a task is updated by the team manager the approval of the task goes to
the Group Program Manager. Please let me know what is wrong?

We are using 2007 version of Project Server

Ravi --

By chance, did the Group Program Manager create and publish the project
initially? If so, the Project Manager needs to open the project, display
any task View (such as the Gantt Chart view), temporarily insert the Status
Manager column, set him/herself as the Status Manager for each task, and
then republish the project. Let us know.
