Hmmmm, wonder if it could be a corrupt .plist. Delete this file:
com.microsoft.entourage.database_daemon.plist
Also check in System Preferences: Startup Items: for duplicate Microsoft
Database daemons. If there are more than one, delete all and Entourage will
create a new one.
To eliminate your User's folder, you could drag your Microsoft User Data
folder to the Shared folder. Create a new User in System Preferences. Switch
Users. Drag the Microsoft User Data folder (it copies it) to the new
Documents folder and open Entourage there. Test reminders. Do they work in
the new Identity? If yes, then it is most likely a preference file in your
regular user's folder. If no, then we need to eliminate your Identity.
Create a new Identity in Entourage and test. If it is database corruption,
you could try rebuilding your Identity. Make a backup in the Finder first.
How to rebuild:

Don't forget to move the Microsoft User Data folder back from Shared when
you are finished testing. IMPORTANT
