tear off stub

Does anyone know how to create a PUB that has 4 pages book (booklet) style
that has a tear off stub, say on page 1 and two? I received one and now I am
trying to re-create my own.

The tear of stub even has dashes top to bottom as a helpful location as to
where to start to tear stub off. Anyone ever produced one of these with
Publisher?

This stub was called a "Response Form" with text on both sides.

Any help would be appreciated.

Booklet setup, create a table, use dashes for the lines in the table.
You could setup the table on the master page so you can see where to place the
text on both sides.

I did book, page setup>booklet 5:/8". Created table on the right edge of
first page but do not see in format table where to make line into dshes.

Draw the lines manually, select the line, use the line dialogue. It maybe easier
just to create rectangles. Select the rectangle, click lines, more lines, you
can select a side to have no line or a dashed line. I had forgotten you couldn't
designate dashed lines on a table.
