template family tree w/space to add medical death/date

I cant find a template that I can use like the family tree, but with wide
enough spaces to add type of death (medical, etc) and date=this is helpful
for the family...especially genetic info. Can be made to only go back to
great or great-great grandparents. Anyone know where to find something like
this?? thank you, LB
