template for Avery C32011 business cards for Publisher

Does anyone have a template for the Avery C32011 business cards I can use in
Publisher 2003?

I have tried to set it as a custom document with the spacing but it still
does not work

In Publisher's page setup, click change copies per sheet and input the numbers you
see in the above link. This dialogue is available when you go to print in earlier
versions of Publisher.
