Template for Cataloguing All My Credit Cards

Does anyone have a template for recording their credit cards. I am always
worried that I am going to lose track of one or more of them.

It would be great if someone designed a template for cataloguing credit
cards. In the old days, we were advised to photocopy the back and front of
each credit card, and carry the coopies in our luggage or on our person, in
the event we lost a credit card. It's hard to imagine such advice today.

If I design it myself, I will submit it to Microsoft templates for inclusion
on the list. I want to create a list for my husband and one for me. Each
listing should include:
--Name of credit card issuer
--Name of credit card holder (exactly as it appears on the card)
--Account number
--Expiration date
--Mailing address of issuer
--Telephone number of issuer to report if lost or stolen
--website of issuer

Of course, one would not store such a list on his/her computer.

I made one and will submit it to Microsoft and will also upload to my
website 5/13/07. It is Excel 2007 format
