Template for Pet Records

I am looking for a template that I can use to keep track of vet visits,
vaccinations, etc. for my SIX pets!!! I need something to get more more
organized than just a stack of vet bills!

I know for a fact that Works Suite used to have exactly that. It was a Works
spreadsheet file. You might be able to find it online. If not, ask some of
your friends, I'll bet they can send it to you. It came with Works Suite
2000 but I'll bet it's still available.
