Template For Phone Messages In Outlook or Office?

My boss gets many phone calls a day and we still use the old written phone
message book and many days we have 30 or 40 little paper messages and it's
become unbearable. I'm looking for a way to display all incoming phone calls
to my boss, hopefully in outlook so it's all in one location. Any
suggestions?
