Template for recording door keys please

I am after a template to record use of door keys. Our office gives out house
keys to carers going to client's houses and then returns them when they've
been so want a template to document names & signatures etc - any ideas?

Keep it simple. A basic spreadsheet with the following data:

Key number or ID, print name, sign name, date out, date in.
