Template in Outlook

I used to inform in our I.T. department about problems in computer to solve.
To speed up the work I would like to use fill up form system in outlook
template.
Could you please inform me how to create template in outlook. I want to just
fill up the template each time as : Employee Name, ID Number, Department,
Contact Number.

Would appreciate your early response.

I have published a form and saved. When I go as below I can see that saved
form:

To open published form email : I go to Tools, Form, Choose Form
then in Look in : Personal Form Library
Here I can see the folder I made ( Published)

But once I send to somebody else They cannot read what I filled up in
message area. In the form I have fixed one label box and text box. Both are
not visible once that receive to receipient. How can I solve this? would
appreciate your valuable information

Did you click the Edit Read Page button and put your controls on the read layout?

You will also need to check the "Send form definition with item" checkbox on the (Properties) page so that your recipients will have access to the form layouts, embedded in the item.

Dear Sue,
Great! Now I can see Label and Text in recepient's mail. I could not see
the text what I typed in text. How come not visible once I sent it to
recepient? So please let me know how can I see the test what I typed to send
and once access it by recepient could not see

Another thing is :
I saved this template in my desktop too. I dont know what is the extension
of this file. Once I click this I can see the perfect email template I made.
I tried to edit
this form, saved and closed. When I open again I could see changing not
effected.
Why? How to edit? Would appreciate your great response.

Look on the Properties dialog for the text box control, at the top of the Value tab. You should see there the name of the property that stores the text the user types into the control. If you see no property, you'll need to set one. Otherwise, Outlook will not know what field to store /read the data.

In edit mode I can see 2 areas. 1) Edit Compose Page 2) Edit Read Page
From where I have to go to Value tab? I nee more explanation. I am
confused.

I have tried a lot from receipient side to access the sent mail. But
recepient getting without any text typed from sender. Please tell me in
detail what is the reason?

You need to look at both page layouts.

If you want the user to enter data in a control on the compose page layout and be able to see that data in a control on the read page layout, then both pages need a control that is bound to the same Outlook property. A control is a text box, check box, etc. Right-click any control and choose Properties to bring up its Properties dialog. The Value tab of the Properties dialog contains the information on what field (if any) is bound to the control. Look at the top of that tab.

Not working. I have tried. Please tell me : For example I put a label
"Name". I need a text for this label. What is the relationship between
the text and label? Shall I give similar name for a text same as label? I
need your great favor on this.
Please tell me where is the problem

A label is a control that is usually used to display static text, in other words, text that is always the same. You enter that text on the control's Properties dialog, under Caption.

In some scenarios, a label control might be used to display the value of an Outlook property. In that case, on the Value tab of the control's Properties dialog, you'd click Choose Field and select what property you want the control to display. You would not need to enter a Caption value on the Display tab of the Properties dialog, because the label's text would come from the property you choose in the Choose FIeld list.

It is more common, though, to use a text box control rather than a label control to display the value of an Outlook property, using the Choose Field button to select the property whose value you want to display. You can also drag a field from the Field Chooser to a form page and have Outlook create both a text box bound to that field and a label control whose caption is the name of the field.

Let me put it this way. I want to send a mail as below :
To, From, Subject label and field is already there.
I would like to create as a fill up form as below :
Name : " "
Tel# : " "
Address: " "

How can I do that? After creating this many time I will take this format
and fill it up.
Then I will send this. Once I want to send the same again and again after
filling up optionally Name, Tel# and Address how can I send? As what file
and where I can save this template to use later on always? I am in failure
to do this. Please help.

I have told you how to do it several times now. Did you follow the procedure that I described, placing a text box control on both the compose and read layout with the pair bound to the same Outlook property? You need to repeat that procedure for each bit of information that you want to capture. If you don't understand what a field/property is, see http://www.outlookcode.com/d/fields.htm.

Once you complete your form design, you can use the Tools | Forms | Publish Form command to publish it to your own Personal Forms library for testing. Once you complete your tests and have it working to your satisfaction, we can talk about how you can make it work sending it to other people.

Yes, I have followed the procedure that you described. I know what is field
property. I have completed form design and published the form to Own
personal form library. How can I make it work sending it to other people?
