Template - with fill in fields

I am new at this. I have created a form with fill in fields. I want the
client to be able to fill in a field and "freeze" the document so that when
they fill in a field the next question on the same line does not "move"?
Does that make sense?
ie NAME:_________________________Date:______________________
so when they put in the first answer to "Name" it only allows say 35 spaces
and as they type their name the "Date" field will not move.....

Word?

Use form fields (not underlines), and then use document protection to "lock"
your document for "forms". If you post back with the specific version of
Word, we can be more specific. Here's an article for Word 2003:

http://office.microsoft.com/en-us/word/HP010446741033.aspx

Basically, you protect it from changes EXCEPT in the form fields, where you
want them to type.

You can use tables to keep some of the fields from moving, so overage will
wrap within a table cell.
