Template Wizard not creating a record

Hi all,

I've built a user survey using the Template Wizard add-in.
Two users have reported that when they hit the Save
button, they are NOT prompted to create a new record. They
are taken directly to the Save dialog box. (I also notice
there are several blank lines in the database file.) Any
ideas why this function would not be working for some
users?
