Template Wizard with Database Tracking

Does anyone know of a good resource for this subject? I
am having so much trouble with this function. Every time
I think I have it all set up I get this error
message "Template Wizard cannot read database" when
saving a file to be added to the database. Thanks to
anyone who can help.
