Would anyone know why when I create a template with visual
basic controls the code is saved in the .dot file but a
new file based on the .dot contains the controls but none
of the code?

Because that is the way that Word is designed.

The code is accessible as a result of the document being attached to the
template, provided that the template is present on the PC where the document
is opened.
