Templates and Outlook Contacts

I'd like to create a MS-WORD template for a customized Fax
Cover Sheet. This template would allow the user to access
records from his/her Outlook Contact list, and then merge
specific record information into the template. Can someone
point me in the right direction or at least shed some
light on this... Thanks in advance.
