We are setting up a document in a Electronic Medical Record program that will
have patient demographic information linked from the database that will
populate in the header.

Then there is a table with 1 line/column for nurses to fill in phone
conversations . WHen setting it up as a form it goes directly to the cell
we want them to use, but when locked the demographic information that is in
the header does not automatically populate. Does anyone know the what is the
best way allow the demographinc information in the headerto automatically
fill in, but keep the cell in the table as the bookmarked starting point but
still allowing the information to populate in the header?

You give no information on how you expect the information to be linked from
the database or for which patient the form is being filled out.

I would have a userform in the template that contained a combobox which was
populated with the patient names and the user would select a patient from
that combobox.
