I've been trying to create a template whereby a client can write in the
blanks without changing the format/layout of my template. I keep
saving my document as a template but when I go back and try and write
on the form I've created the format and layout changes as well. Can
anyone help?

If you want a "type in the blanks" form, the easiest way is probably to use
a table. But you will find a lot of suggestions/instructions at this
website--they are geared for WinWord, but should translate over fine, if you
get stuck on anything, feel free to post here.

(hit refresh a few times in Safari, or use a different browser)

The page first has links for Word's on-screen form feature--that's probably
only necessary for fairly complicated things, and I think there may be
significant Mac/Windows directions in that system. So move past those links
to read the the rest of the page, which discusses how to use Tables.
