The operation failed - phone number

Every time I enter a phone number, any kind, I get this
error. I have run updates and repair. Noting seems to
help.

It does not matter in what format I enter the numbers i.e.
####### or ###-#### or ###-###-####. The message is the
same.

The data is saved but the numbers do not format into the
normal mask. (###)###-####

Any ideas?
