Try Google to find a template. But MLA and APA demand things like page
numbers in a certain place, etc--that's the type of stuff a template
handles. I use Chicago myself, and I don't recall it saying anything
about layout. A template is not going to handle doing the bibliography
in Chicago style, if that's what you want. See the "#1: Introductory

The new Windows Word 2007 now has a citation manager, but as I
understand it isn't sophisticated enough to really handle the needs of
dissertation writers.
