timesheet totals do not add up columns correctly?

we have had a few cases here recently, where an individual will fill out his
timesheet, and the total at the bottom of each column and the cumulative
total at the bottom of the page do not add up to the sum of the values in the
actual work for that column.

For instance, Monday, individual puts in 2hrs on task 1, 4 hrs on task 2, 1
hr on task 3, 1 hr for admin...should equal 8 hrs...in some instances, this
will show 12 hrs (can't really say for sure, but it doesn't equal the 8hrs)
and as we complete the timesheet for the week, his 40 hr week, ends up
looking like 64hrs.

any ideas why or what may be causing this anomaly? as it is very puzzling.
we delete the timesheet and recreate it, same behavior.
