Timesheets pending approval

We are noticing that, occassionally, a single task (not all of the tasks
submitted) from a Resource's timesheet from two weeks ago (or more in some
cases) will re-appear in the Resource Manager's approval que, after it had
already been approved two (or more) weeks ago. I'm wondering:

(1) Is this a known problem with MS Project Server 2003, SP1?

(2) The description for fix Number 107 on Service Pack 2a says "When a
resource submits an update for a timesheet, all the previous timesheets
appear again." The description is slightly vague. Is this fix designed to
address this problem?
