Title bar disappears after Lock/Unlock of PC

If I have my Outlook Client and/or an email open (maximized) and I
Ctrl-Alt-Del and Lock Computer, when I unlock the computer the Title Bar is
now missing from the Outlook Client and/or the Email that was open
(maximized) at the time. If I minimize the windows and then maximize them
again, the Title Bar comes back. Any ideas?
