TOC and Index for multiple docs

I have large, complex docs written in Word 2003 where each chapter is a
separate Word file. I need to create a TOC and an Index for the entire
document. I can't use RD fields as they do not create hyperlinks. I can't use
INCLUDETEXT fields as the docs are too large to make that time feasible.

The best solution would be to use another authoring tool like FrameMaker or
an XML editor but that's not an option for these documents. Does anyone have
any suggestions on how I might accomplish this using Word?
