TOC and page numbers

I am currently working on a document and having
difficulty with a glitch in the TOC. This problem occurs
for both TOC2 and TOC3. While the style is the same for
all of these headings (left margin = 1, right margin =
5.99, with leader), 3 of the headings do not work
properly. For instance:

TOC 2..................67
TOC 3..................68
TOC 2 69

What could be causing this problem when the tabs are
correct?

the entry could either be very short or larger than one line, in any
case it ends before a <tab> entry before the flush-right tab (it might
be there even though it's not in the style, mind you, see:

Whenever I update my Table of Contents it acquires unwanted tabs, and I
have to press Ctrl+Q to get rid of them (by Dave Rado)

Just to follow up, I changed the tabs and all is fixed!
