TOC does not open properly in e-mailed doc

Several of us have tried to unravel this one. In the user's original
document, the TOC (based on headings) appears correctly. We update the
table to make sure it is so, and all is well. The document is saved and
closed. The user then attaches the document to an Outlook e-mail form.
When she goes to the Sent mail folder and opens the attachment via
double click, the TOC appears with a single page number for all the
heading entries. If the table is updated, it corrects. When the
document is saved from alternate click, then opened, it displays
properly. The problem is that some of the intended recipients are way
below the bar to update the TOC.
Any ideas?

Reading Layout view is known to corrupt the TOC. The recipient should go to
Tools | Options | General and clear the check box for "Allow starting in
Reading Layout."
