TOC Header 2 adding "source" in listing

I'm trying to update the TOC my team has been using for ages.
Originally, the goal was to add chapter numbers as a prefix to the page
numbers. After changing the built-in styles around, I accomplished
that. Now, as I'm changing the TOC around to look for my updated
styles, they all look good except Heading 2. This is what it looks
like:

Source.............................................................O
verview of Chapter 1.......................................2-1
Source.............................................................C
hapter 1 Introduction......................................2-2
etc.

The weird part is that "source" isn't shaded like the rest of the text.

Any ideas or suggestions would be appreciated.

Examine the formatting of the TOC 2 style. Most likely the "Source" prefix
has been added as "Numbering" formatting.

Yes, you were correct. I have no idea how it got in there, but after
examining "Modify" and TOC3 style, that style sure enough had "source"
in the numbering. Bizarre!

Thanks, that was driving me nuts!
