TOC issues using RD codes

I am using W2007 and have RD codes set to build a combined Table of Contents
from two “chapter†files. The combined TOC is defined in a separate front
matter file. I discovered that the TOC is repeating Chapter 2 headings.
Further, the repeated entries lose their chapter number definition with the
page number and the repeated Heading numbers start at 1 and 2 instead of 2
and 3 that they actually are within the chapter file.

The TOC entries look like this:

1 INTRODUCTION 1-1
1.1 Purpose 1-1
1.2 Limitations 1-1
1.3 Commitment 1-1
1.4 Proprietary Data 1-1
1.5 Document Change Procedure 1-1
1.6 Document Structure 1-2
2 INTRODUCTION 2-1
2.1 Scope 2-1
2.2 Commitment 2-1
2.3 Proprietary Data 2-1
2.4 Document Change Procedure 2-1
3 SYSTEM OVERVIEW 3-2
3.1 System Design 3-2
3.2 Business Objectives 3-2
1 INTRODUCTION 1 REPEATS START HERE
1.1 Scope 1
1.2 Commitment 1
1.3 Proprietary Data 1
1.4 Document Change Procedure 1
2 SYSTEM OVERVIEW 2
2.1 System Design 2
2.2 Business Objectives 2

I made sure that TOC options are set to only pull in Heading styles
(originally, that was not the case). But when I update the TOC, I continue to
get the repeated headings and they continue without the chapter/heading
numbers.

This combined TOC file is only reflecting the first 2 chapters; ultimately,
it will hold about 15 chapters, so it is imperative to get this figured out.
This will ultimately be delivered to a Customer electronically.

If anyone has any idea on how to remedy this, I would greatly appreciate it.
I feel like the Twilight Zone is visiting me!

Never mind...I found the source of this problem. The worst part , it was
self-inflicted! I negelected to strip out all the headings and content in my
front matter file. I just needed to walk away from my desk to see the problem
with fresh eyes.

Mea Culpa to anyone pondering this.
