TOC wouldn't update page numbers??!

My TOC is tied to one Heading style. My field code is
{TOC \o "1-1" \h \z}.

I couldn't seem to get the page numbers to update correctly. I
tried :
-- right-click, Update Field, Page Numbers Only
-- Alt+F9 to show codes, then Alt+F9 to show results
-- Print Preview and Close

Finally, I had to right-click, Update Field, Entire Table, which threw
off some tab spacings that I had to go back and fix, to get the right
page numbers.

Why wouldn't Page Numbers Only update the page numbers?
