Toolbar missing

Word 2000
When I load certain templates (my own...) to run a document from, a
button I have placed in the toolbar to run the opening Macro (which
brings up a UserForm) is missing.
In some saved versions of the Template it comes up OK in others it's
not there.
What am I doing wrong.

in some versions you may have not set the context correctly whilst
mucking with the toolbar and those changes were stored in normal or
another template.
