Toolbar starts hidden

Hello,

I have a COM addin written in C++ that adds a command bar with 2 buttons on
it. Everything worked fine for a while until I added a third button. Now
the addin loads but my command bar is hidden. When I right click on the
toolbar, I see my addin listed in the menu. When I select it, the buttons
appear and function properly.

What happened and why don't my buttons appear?
