Traceability

Does anyone know how to make traceability into a set of
word documents? I mean the next:
1.- I have a list of phrases to trace
2.- I have a set of documents to look into
3.- I need to have a matrix with the phrases as columns
(or rows) and the documents as rows (or columns) with the
cells filled with the references (table of references)
from the documents.

If anyone know a tool or some hint please help me
