Tracking assets containing three barcode fields

Is anyone familiar with an Access template for tracking assets that contain
three barcode fields? The three fields we work with are model number, access
card number and serial number.

The assets are receivers for DIRECTV satellite television for which we have
a team of installing technicians operating in several warehouse locations
that are replenished by multiple suppliers. We would like to track these
receivers after receipt from the supplier to the installation at the end user.

Barcodes mean a means of reading them. These devices usually fit
between your keyboard and your computer and mimic someone keying in
the code directly. USB bar code readers plug in separately but look
like a keyboard.
