Trouble TOC

I am working on a project that has about 10 chapters. Within each chapter
are titles that I would like to have in the TOC.

The problem I am having is some of the TOC's are not showing the leader from
the end of the title to the page number. This is only happening in two or
three of the chapters.

As far as I can tell the settings are all the same.

Oh yeah I using Word 2003 with Win XP.

I use the insert>reference>index and tables command to start the TOC when
prompted I check change all not the page only numbers.
