Troubleshooting custom keyboard assignments.

I recently customized my keyboard assignments. Now when
I use my Alt key to open items on my menu bars, a few of
them will not open. (Insert, Format, & Tools)I opted to
go back to the default settings. My Alt key still will
not work on the same items.

What application are you referring to? How did you customize the
keyboard assignments?
