Trying to use forms in word 2000

I am trying to create a form for our INTRANET page. I am
using Word 2000 to do this.
I believe I have the form created correctly.
The problem is when I submit the form the data does NOT
go into the .txt file that I want it to.
The best that I can tell it does not go anywhere.

If anyone has any suggestions on this I would greatly
appreciate it.
