Unable to add a command to a toolbar.

Using the VIEW > TOOLBARS > CUSTOMIZE dialog I am unable to drop certain
commands onto any toolbar even though I get the "+" sign indicatiing I should
be able to execute the drop.

In particular I tried to put the "Save as source files..." command onto the
standard toolbar in MS InfoPath. I am able to drop other commands onto that
same toolbar.

What is causing this behavior?
