Unable to Initialize Business Contact Form

I don't know what I did but this message appears in the "General" tab when I
try to edit or creat a Business Contact:

"Business Contact Manager was unable to initialize this form"

The "Details" tab does not have any problem.

I reinstalled Business Contact Manager but that didn't solve my problem.

Help!
