Urgent BCM problem

When I try to open any account in BCM, I get the message "The custom form
could not be opened, Outlook will use an Outlook form instead...". When
OKed, I get the message "array index out of bounds". When OKed, the account
is opened in an Outlook form with crucial information missing.

I have tried Maintenance/Start to rebuild indexes, no change. Also tried
restarting the PC (which have actually helped before with similar problem).
I have a backup that can be restored, but that is last resort since I will
be losing data.

Any suggestions?

I have also tried the tip from www.slipstick.com Tools | Options | Other |
Advanced Options | Custom Forms, then click Manage Forms, then Clear Cache.
This clears the problem for some accounts only.

Clear the forms cache. Delete everything in this folder: C:\Documents and
Settings\[user]\Local Settings\Application Data\Microsoft\Forms
