Urgent help! - Regarding Outlook From question - Attachement!

Hi!,
We do have a custom created outlookform. When ever we send this form to
others, an attachement is also been added. We do want to remove the default
attachement thats being added when we send the custom outlook form.

Can you please help me/guide me to remove the attachment being added in the
form?

Please advise.
