Urgent help with formatting issue needed please

I am trying to format a report in a specific way. What I want is this:

CODE Date1 Name1
Address Date2 Name2
City, State, Zip
Phone

What I am getting is:

CODE Date1 Name1
Address
City, State, Zip
Phone

CODE Date2 Name2
Address
City, State, Zip
Phone

I have found how to hide the second set of CODE, etc, however the space is
still ther and I need the Date2 Name2 to be directly under the Name1 Date1.
Any help would be appreciated.
