URGENT - Need access to Form Text boxes on document received via email

Hi,

I need to complete a form and it has form text boxes and
check boxes. I cannot type anything or delete the words
{FORMTEXT} that appear.

Can anyone help me so I can complette this form TONIGHT!?

Press Alt+F9 to toggle off the display of field codes.
