user defined fields and forms

I have two problems with custom fields and forms in Outlook

1) I have made a custom contact form with 3 user-defined fields. I would like to add these three fields to the "frequently- used fields" list/category. Is this possible

2) My contacts have data for the 3 user-defined field, but still open up in the defaul Outlook contact form. How do I transfer these contacts to the new form
