user-defined fields and vb script

Maybe I chose the wrong time of the year to ask so I'll try again.

I have recently managed to design and publish my first form. It is based on
the task form and is called "work_request". I filled it with about 20 user
defined fields. After publishing everything worked fine and
I was asked to add four more fields. After adding the four new fields to the
form and republishing it I noticed that the new fields appear in the "field
choser" box under "user defined fields in folder" while the 20 orignal
fields that I created all appear under "work_request" in my "field choser"
box.
This is not an ideal situation. What can I do so that all 24 fields will
appear under the same heading ?

I would also like to add some intelligence to my form.

1) my form has 6 frames and I would like to hide some or make some read only
when the username of the person looking at the form begins with "ABC".

2) I have a field called "status". When its value is changed I would like to
set the value of the my field "updater" to the username of the person who
updated the status and to set my field "update_time" to the current time.

I would like some idea as to how to script these 2 functions. My knowledge
of vbscript is
minimal.
