Userform is no longer invoked

Hi:

I am having a problem with a userform that is no longer invoked once the
custum button is pressed. What it is call up a template with the userform to
enter specific fields and then print two copies.

What did I do to cause this issue?

Possibly, you have increased the security settings so that macros are
automatically being disabled.
