Using a Pop-up Form As a Search Tool

I would like to incorporate a pop-up form and enter search
criteria into designated fields, and in return have a
report showing the requested infromation.
Ex. Say music search
Search1 Country/western
Search2 Slow
Search3 Bo James
If anyone can help I would really Appreciate it

I Would Like to search by the following fields:
Recording Artist
Music Category
Recording Title
Track Tempo
Track Specialty
Track Title
