Using form with Outlook 2003

I have create a new form based on the contact forms. When i create a new
contact based on this new forms there's no problem but when i try to send
it to another persons it takes the outlook contact default forms.

Did someone have a solution for this problem.

What, precisely, are you sending the other person? If you want to send them a copy of your form, you'd need to save the form as an .oft file and send them that.
