Look on the MVP FAQ site for the page on how to create a userform. Also,
look in Word's help for "online forms" which have what you want except for
the radio buttons.

I would like to create a form that has radio buttons, checkboxes, and
places to fill-in text that would then generate a text document. I have
done this before in Filemaker, but do not know if it is possible in Word.
If not, is it possible in another Office program? Or, can anyone advise on
another way to do this in Word? Basically I need to create documents
wherein most of the information remains the same, but certain words change
and some paragraphs/bullet points may or may not be included depending on
the individual person the document is intended for. Thank you!
