Using Junk Mail Rules

I have Office 2003 and have the setting for the Junk Mail to Safe Lists
Only: Only mail from people or domains on your Safe Senders list will be
delivered to your inbox. I changed it to that recently because I seemed to
be getting heaps more spam and was fed up with going through it all. But I'm
finding that it's not working. People who should be on my safe list and
from whom I've been receiving mail regularly for ages keep getting dumped in
the Junk Mail and I'm getting heaps of junk mail about PHARcsMACY and
similar going into my inbox from made up addresses that aren't in my safe
senders list. It seems somewhat topsy turvy and is not working. I've tried
the high setting too but that's not working either. I also get lots on
Stock Alerts that end up in my inbox and not in the Junk Mail.

Why isn't it working like it should?
