using sample survey form

I'm new to using forms so I thought I would download the sample survey form!
I've done that and unzipped it.

When I type a question in and click Add, nothing happens. What am I doing
wrong? I think I followed every instruction given!

Did you publish the form? With the Send Form Definition with ITem box
unchecked?

Thanks - that was the missing step! Isn't it great that this stuff isn't
documented or at all intuitive - I make a living out of teaching lots of
Outlook skills to improve workplace productivity!! Without some guidance,
it's just too hard to find all the useful stuff Outlook can do.
